Two repeat citations cover failure to assign a competent person for daily trench inspections and leaving workers in hazardous conditions. Three serious citations address missing hazard training, inadequate egress, and materials placed too close to the trench edge.

Federal investigators cited T&T Pipeline Co. Inc. of Orlando after a worker was fatally struck by a rotating pipe stem during a horizontal drilling machine drill bit change at a New Smyrna Beach jobsite. OSHA classified the struck-by exposure as willful, finding the employer knowingly failed to control rotating equipment hazards during a standard maintenance task. Struck-by incidents involving rotating and moving equipment remain among the leading causes of construction fatalities.

Federal investigators cited the general contractor and five subcontractors at the Monroe Commons mixed-use project in Monroe, New York, after finding an unstable embankment nearly 75 feet high that was not adequately sloped or supported. OSHA issued 28 citations, including five willful and 23 serious violations, covering cave-in protection failures, inadequate daily excavation inspections, and chemical exposure hazards. The multi-employer case is among the most significant construction enforcement actions in the New York region in 2026.

Federal investigators cited Always Underground LLC of Frankfort, Illinois for a fifth repeat violation after workers were found in unprotected excavations during utility construction. The citation was issued under OSHA's National Emphasis Program on Trenching and Excavation. Repeat classifications reflect a pattern of knowing, persistent non-compliance and carry penalties up to ten times those assessed for initial serious violations. Trench and excavation cave-ins remain among the most consistently fatal hazard categories in construction enforcement.

OSHA's September 8 QuickTakes outlines an expansion of the VPP framework from four to seven safety and health management system elements, the first structural update to the cooperative recognition criteria in decades. The new framework aligns more closely with OSHA's Safety and Health Program Management Guidelines and gives high-performing employers a clearer pathway for achieving and maintaining Star and Merit status, with modernized recognition programs for facilities with best-in-class safety cultures.

OSHA pushed the employer deadline for updating workplace labels, safety data sheets, and worker training from July 20 to November 20, 2026. The revised Hazard Communication Standard aligns with GHS Revision 7, adding new categories including Desensitized Explosives and Chemicals Under Pressure, covering all industries from construction and manufacturing to healthcare and energy.

Four workers employed by subcontractor Shang Hao Machinery died after falling from steel framework at TSMC's under-construction Fab 22 facility in Kaohsiung, Taiwan on October 20. Preliminary findings point to bolt failure on a third-floor exterior steel connection plate. TSMC voluntarily suspended steel structure work at Kaohsiung and all new fab construction sites across Taiwan pending the investigation. The Southern Taiwan Science Park Bureau stated that construction will resume only after the employer submits an improvement plan that clears regulatory review.

OSHA joined four national safety and industrial hygiene organizations in a new alliance targeting serious injuries and fatalities across construction, general industry, and maritime. Partners will develop shared messaging, educational resources, and training programs focused on high-energy hazard recognition and precursor identification. The alliance formalizes a coordinated approach to the hazard scenarios that account for the most catastrophic outcomes, with an emphasis on supervisor accountability and field-level behavior change.

Jordan Foster Construction signed an OSHA alliance to protect workers building a 70-acre Public Safety Training Academy campus in El Paso, Texas. More than 1,300 workers at over 100 Georgia jobsites joined the Georgia Utility Contractors Association's trench and excavation safety stand-down, reviewing competent-person requirements, hazard recognition, and best practices for the trenching fatality category that consistently leads construction enforcement totals.

On April 24, 2026, PHMSA issued 40 notices of proposed rulemaking, final rules, and direct final rules updating Parts 191, 192, and 195 of federal pipeline safety regulations. Thirteen final rules affecting gas pipelines, LNG facilities, and hazardous liquid operators became effective August 3, 2026. Fifteen direct final rules incorporating new ANSI and API industry standard editions take effect January 1, 2027. Operators of onshore LNG facilities should verify compliance status across all applicable actions in this rulemaking package.

OSHA's July 3, 2026 regulatory agenda confirmed a Supplemental Notice of Proposed Rulemaking for the heat illness prevention standard by December 2026, with a final rule projected for October 2027. The standard would cover general industry, construction, maritime, and agriculture, requiring written heat plans, monitoring at 80 and 90 degree F triggers, mandatory rest breaks, and documented acclimatization protocols for all new and returning workers.

California's Division of Occupational Safety and Health cited drilling contractor Drill Tech Drilling and Shoring and engineering firm Provost and Pritchard Engineering Group after a 35-year-old geologist was killed and a second worker seriously injured in the April 14 collapse of the Canyon Tunnel near Knights Ferry. Investigators found both employers permitted work to continue without removing or adequately supporting identified dangerous ground approximately 200 feet inside the tunnel. Provost and Pritchard has appealed. Cal/OSHA's Bureau of Investigations is conducting a separate parallel investigation.
